Enhanced Care Plus is a primary care initiative funded by the Ministry of Health. The programme is designed to enhance care for patients who have high health needs with one on one consultations with a support nurse. This will help empower people to manage their chronic illnesses. A designated nurse will work with you to help you achieve your goals. Goals are set and a care plan is developed for each patient. There is a strong focus on education and preventative care. This service is FREE to patients. The funding allocated allows for four nurse consultations and one telephone review.
People are invited on to this scheme dependent on the level of need.
